LSO Forms and Documents for Coaches
Little League Background Check form The Little League background check is required for all coaches and assistant coaches in all levels of baseball. It can only be completed in paper form, not online, and must be handed in to league officials. There will be many dates available to turn in forms, please check your email for a list of dates. The form does require a social security number and will be handled with care and shredded once entered. A copy of the applicant's drivers license is also required.
